When you arrive for your initial consultation, you will be given one form to complete. To expedite this process you may print and complete this form in advance for your appointment, click here to print.

Included within this form is six pages; Page 1-2 includes a consent form designed for collection, use and disclosure of personal information, Page 3 consists of residence/address and insurance information, Page 4 is a thorough medical history and listing of serious ailments, Page 5 lists your dental history, and lastly Page 6 is a complete head and neck pain evaluation tool. To ensure that we have the most up-to-date information concerning your general health, your medical history will be reviewed (and updated, if necessary) annually during a visit with your dental hygienist or dentist.

Should your medical conditions change, particularly with respect to any medication you are taking, please inform us prior to, or during your next visit.

During your first visit your dentist will go through an initial series of radiographs (X-rays) and examine your oral health then discuss any recommended treatment and together make a treatment plan with you. You will also meet with one of our dental hygienists after who will provide information regarding preventative care and gum disease. A thorough dental cleaning will follow this consultation. Upon the completion of your visit, our staff will assist you in setting up future appointments.

 

Your initial visit is just the beginning of an active partnership towards a healthy and more beautiful smile. As such, we want to assure you that you can feel comfortable about asking our dentists and staff any questions you may have.
